It is expected that the service will deliver the following outcomes (based upon the agreed risk stratification) to support a reduction in the risk and incidence of infections in the local population as a whole:

·         All C difficile and MRSA infections will be investigated collaboratively with health and social care partners as required.
·         Other significant infections e.g. CPE will be investigated collaboratively with health and social care partners as indicated.
·         All nursing homes will be audited against the revised audit tool
·         All residential homes will be audited against the revised audit tool
·         All GP practices will be audited
·         All dental practices will be visited and progress on self-audit (key concerns/ successes) reported
·         Six infection control sessions will have been delivered to care home staff
·         Infection control training will be offered to primary care staff at each visit and documented
·         Support to embed the NPSA cleanliness tools will be offered to all care homes, GP and dental practices.
·         HMP Kennet will be audited twice and visited quarterly
·         A network of link champions will be established in all GP practices
·         A network of link champions will be established in all residential homes

The Service will deliver seamless care with other providers at all times, to ensure that infection prevention and control advice and support is standard across the wider health and social care economy. The service will work to all relevant Public Health England (PHE) protocols for contact tracing, incident and outbreak management.
 
The service is funded to provide a service to support Sefton Local Authority, South Sefton and Southport and Formby Clinical Commissioning Groups (CCGs) and NHS England in its responsibility around HCAI reduction and direct commissioning of Primary Care.
