The CCG is looking to develop a Care Home with Nursing for Service Users with Complex Dementia Presentations. The facility will offer 18-22 beds and the contract is for a period of 5 years with the option to extend for up to a further 2 years.
The commissioners are seeking a service that can be within an existing building (but must be as a separate element not just the beds in the general home) or new build both of which will offer the right environment to support this complex client group.
The Provider will be contracted to Tameside MBC Pre Placement Agreement for Residential and Nursing Care with an additional NHS Standard Contract 2017 and 2018/19.

The provider will be commissioned to deliver innovative, high quality, outcomes focussed, personalised care in an environment and culture that allows interventions to be delivered in the least restrictive way. Partnership working across the health and social care sector and a workforce that has the skills and understanding of those with complex and challenging needs will also be key to a quality service.
The levels of needs and support will be over and above those within standard residential nursing and care.
