Trafford Council and NHS Trafford CCG are determined to ensure that all carers are able to access a range of personalised services to support them in their role and to help the people they care for remain independent and within their own homes and communities. Fundamental to this is ensuring that all carers are identified early and have access to a quality carers assessment and carers support plan that recognises and values their caring role and the contribution they make but which also identifies their outcomes enabling them to have a choice and control.
The provider will deliver a community based service that will ensure all carers of both adults and children in Trafford are identified and can receive a carer’s assessment under the statutory duty of the Council where appropriate.
