Trusted Assessor Service – Cheshire – Soft Market Testing Survey
This service will initially work with existing care home residents who have been admitted to hospital and require assessment prior to transferring back to the care home.
Cheshire East Council is conducting a soft market testing survey to inform the future opportunity of providing Trusted Assessor Service.
The overall aim of this service is to develop and establish a trusted Assessor service in Cheshire East, this service will provide a trusted assessment function through Independent Transfer of Care Coordinators. This service will initially work with existing care home residents who have been admitted to hospital and require assessment prior to transferring back to the care home.
In addition to this the provider will also link in with the British Red Cross service and other third sector organisations to deflect demand. The provider will continue to work flexibly and will work with hospitals to support 7-day discharges where possible.
We would look for the provider to have a presence at Macclesfield District General Hospital and Leighton hospital to carry out the trusted assessment and for work to deflect demand.
We would appreciate the input of relevant providers in this exercise.
