Description of the contract
NHS Nottingham City Clinical Commissioning Group intends to appoint a project co-ordinator who has the relevant qualifications to lead on the implementation of a care pathways website. The appointment will be on a consultancy basis. The project co-ordinator will be responsible for the overall co-ordination of the project and population of the website. This will involve setting up systems and processes as well as utilizing existing resources. A key part of the role will be engaging with a range of stakeholders to ensure that latest care pathways are available from the website. Relationships building and networking will therefore be a key part of this role. The project co-ordinator will need to ensure that stakeholders recognise the important role the pathways website will play in implementing any care pathways within primary care. The project co-ordinator will lead on the development of a standardized pathway template and co-ordinate the approval of pathways using the existing committees in place. The project co-ordinator will be responsibility for transferring current pathways onto the standardized pathway template, co-ordinating the approval of pathways and uploaded onto the pathways website. The project co-ordinator will be required to work in collaboration with the technical team developing the website. The project co-ordinator will report to the project steering group. A hot desk will be available at NHS Nottingham City (Standard Court). The successful provider will be expected to travel between multiple locations.
